Home Report a side effect

Tell us if something did not feel right.

Patients, carers, pharmacists and doctors can report a side effect or a quality concern about any Lumen product. Every report is read by our safety desk the same working day.

If someone is seriously unwell, do not wait for us. Go to the nearest hospital or call emergency services first. This form is not monitored around the clock.

01The report

Four things help us most.

The product name, the batch number, what happened and when. You do not need to be sure the product caused it.

  • Your details stay with the safety desk
  • You can report for someone else
  • We may call to ask a few questions

Example form for this demo. Reports in the live site go to the safety desk only.

02What happens next

Your report, step by step.

  1. 01

    Read the same day

A pharmacist on the safety desk reads every report on the working day it arrives.

  2. 02

    Follow up

We may call to ask about timing, other medicines and how the person is now.

  3. 03

    Check the batch

Retained samples of the batch are reviewed or retested in Ibadan.

  4. 04

    Share with the regulator

Reports are shared with the national regulator as the law requires, without your contact details where possible.

  5. 05

    Tell you the outcome

If you would like, we tell you what we found.

03Why reporting matters

Small reports add up.

No medicine suits everyone. Side effects that are rare in trials sometimes only show up when thousands of people use a product. Each report helps pharmacists, doctors and the regulator see patterns early.

Quality concerns matter too: an odd smell, a broken seal, a tablet that looks different. Keep the pack if you can.

04Other ways to report

Use whatever is easiest.

05Questions

About reporting.

Will reporting get anyone in trouble?

No. Reports are about medicines, not blame.

Can I report a product from another company?

Yes, we will pass it to the right company and to the regulator.

What if I am not sure?

Report anyway. We would rather hear about it.

Is my information private?

It stays with the safety desk and is shared with the regulator only as required.

Would you rather talk?

A pharmacist answers the safety line during working hours.
